Community Networks, AccessCNY’s day habilitation program, is for adults with developmental disabilities who are seeking an individualized program within a group setting. Networks is a skill-based program which builds upon an individual’s interests and abilities.
Based on input from participants, staff members develop initiatives that lead to personal growth and fulfillment for the individual. The focus is on building skills, advocacy and being involved in the community. Participants are assisted and encouraged to access community resources to build new supports and relationships.
Individuals may choose to participate in several group activities, such as cooking, music, exercise and socialization groups. Staff members also work with participants to develop or improve skills that may lead to employment, often through volunteering in the community.

Referrals are accepted from individuals, family members, service coordinators, and other professionals. Individuals must be enrolled in the Medicaid Home and Community Based Services Waiver to be eligible for Community Networks.
