Critical Time Transition Support for Youth
Intensive case management for youth ages 11-17 with a mental health diagnosis transitioning out of a short-term hospital stay and back into the community.
Intensive Case Management
Our program supports youth and their caregivers as they navigate the transition between hospital discharge and returning to their homes and communities. Youth and their families receive individualized support from expert case managers, mental health and behavioral professionals, and peer advocates for up to nine months after their discharge from the hospital.
Youth Mental Health Center (Coming Soon)
Set to open in 2027, AccessCNY’s youth mental health center will feature 8 individual bedrooms along with shared healing spaces where youth in the program can meet their recovery goals in a transitional residence for up to 120 days.
Referrals
Youth will be referred by the hospital directly to our program. AccessCNY partners with participating hospitals so that youth receive support while working towards hospital discharge.
